March 10, 1954 — June 22, 2009

ELKHART — Kenneth Wayne Ellsworth Jr., 55, died Monday at Elkhart General Hospital.

He is survived by his former wife, Cathy; two daughters, Chasity and Jessica Ellsworth, Elkhart; and a son, Jarad Shannon, Bristol.

The funeral is set for 1 p.m. Friday at Hartler-Gutermuth-Inman Funeral Home, Elkhart, with visitation one hour prior to services.

Burial will be in Chapel Hill Memorial Gardens.

Memorials may be directed to the funeral home or to Ellsworth’s children.
